Defence Forces Receive Air Accident Investigation Unit ( AAIU) Report .

The Defence Forces have received and fully accept the AAIU report into the Air Corps Pilatus PC 9 Accident which occurred on 12 October 2009 resulting in the tragic deaths of Captain Derek Furniss and Cadet David Jevens. The fundamental purpose of such investigations is to determine the circumstances and causes of these events, with a view to the preservation of life and the avoidance of similar occurrences in the future.

Commenting on the report Defence Forces Chief of Staff, Lieutenant General Seán Mc Cann said:

“I would like to extend my deepest sympathy to the families of Capt Derek Furniss and Cadet David Jevens on the tragic loss of their loved ones. I fully appreciate that the passage of time will not have dulled their absolute sense of loss.

I fully accept the findings of the AAIU report and have ensured that all recommendations in the report have been implemented or are being implemented in order to manage the risks associated with training and operations in the Defence Forces.

While I must ensure that members of the Defence Forces are trained to the highest possible standards in order to serve the Irish people; I am also acutely aware that we must continue to apply the highest international standards of risk management  to ensure the safety and well being of all our personnel.” 

Safe flying practices and the training of Air Corps Personnel to the highest military and civil aviation standards are of paramount importance to the Defence Forces. In recent years, the Air Corps has introduced a comprehensive system of Safety Audits, Crew Resource Management and Risk Assessment.

The men and women of the Defence Forces are the most important resource of the organisation. Although it is impossible to completely eliminate the risk associated with military training, the Defence Forces are committed to ensuring that all necessary safety measures are taken in the training of personnel to deliver the highest quality military capability to the State.

The loss of Captain Derek Furniss and Cadet David Jevens in this tragic accident has been deeply felt by all personnel, and the thoughts of Óglaigh na hÉireann are with their families and loved ones at this difficult time.
